Everybody we support has a unique "person-centred plan". It
describes the outcomes you want, and the way you want to be
supported.
We use proven methods like Person-Centred Active Support. This
ensures staff all work in a consistent way and enable people to do
the things they want, and develop or maintain skills for
independence.
We provide everybody we support with their own "outcomes report"
so that together we can make sure you are getting the help you
need, to live the life you want.
We help people to take advantage of Person-Centred
Technology, which can be anything from a local help-alarm
system to an adapted easy-grip kettle.
